What should I expect on my first visit?

For your first visit to the Bradley clinic, expect a comprehensive evaluation and parent consultation. Families should arrive 15 minutes before their evaluation time.

We reserve the right to reschedule patients who do not arrive early enough to complete new patient paperwork or are late to the appointment. Our therapists evaluate and treat patients back to back. Their schedules don’t allow them to run over their session blocks.

What should I bring on my first visit?

• Patient Forms – (we will email these to you)
• Doctor’s Order (if applicable)
• Any important documentation, IEP, prior evaluations, IFSP
• Insurance Card
• ID

As outlined in our policy and procedures, cancellations need to be made a minimum of 24 hours prior to the scheduled session. It is important to keep track of your therapy schedule to avoid missing your child’s therapy.
