Arrival

Come 15 minutes before your child’s appointment. (We may reschedule patients who arrive late or don’t come early enough to complete new patient paperwork.)

Before your first clinic visit

Complete the Intake & Insurance Verification Form

What to bring on your first visit to our pediatric therapy clinic

Patient forms (we will email these to you)
Doctor’s order
Credit/debit card or bank information to put on file
Any important documentation, Individualized Education Program, prior evaluations, Individualized Family Service Plan
Insurance card (Milestone Therapy does accept private pay and trust cases for home-based therapy. This may include speech therapy, physical therapy, and/or occupational therapy.)
ID, such as a driver’s license
Visit the page of the clinic you’ll be attending for further information regarding establishing your evaluation(s) and first visit.
As outlined in our policy and procedures, cancellations need to be made a minimum of 24 hours prior to the scheduled session. It is important to keep track of your therapy schedule to avoid missing your child’s therapy.
